A six-month pilot scheme to help people as they are discharged from hospital, and to prevent them from going back in, has been described as "absolutely brilliant".
Mark Bradley, from the Red Cross, was talking about the special work of the Crisis Action Team to co-ordinate all the community services it can to keep people being treated and cared for at home.
The pioneering project aims to co-ordinate work of the Ipswich Hospital, Ipswich and East Suffolk CCG, the Red Cross and Age UK.
Mr Bradley said: "Everybody is working towards one single goal."
