Today a disturbing report was published into the self inflicted deaths of six women at Styal prison in Cheshire in the course of one year.
The Prisons Ombudsman has found that there were "serious inadequacies" at Styal and that disciplinary enquiries should be considered for four members of staff.
A day after she had been jailed for non-violent manslaughter, 18 year old Sarah Campbell collapsed in Styal women's prison, and died in hospital later that day. She was a former heroine addict who suffered from clinical depression and her mother, Pauline Campbell, says she should have been sent to a secure hospital, not to jail.
